Tackling delayed transfers of care: A new partnership brings technology and data to homecare service

A new partnership was today announced by Appello, one of the UK's leading technology-enabled care services (TECS) provider, and Bluebird Care, one of the UK's leading providers of homecare and live in care services, to help tackle the growing challenge of delayed transfers of care.

The new service, called Bluebird Care Assist, will see Appello and Bluebird Care develop new care pathways through integrating homecare services with TECS solutions. Bluebird Care Assist will provide round the clock monitoring for people receiving care in their home and will enable people to return home from hospital and help them to remain in their homes living independently for longer.

Through its capability to monitor the health and wellbeing of people receiving care and support services in their home, Bluebird Care Assist has the potential to deliver significant improvements, from utilising technologies to supporting the integration of health and social care.

Duncan Berry, chief operating officer at Bluebird Care, said: "Every day many older and vulnerable people are ready to be discharged and yet they remain in hospital, often delaying their recovery and return to independent living. One of the reasons for this is hospital staff responsible for discharging patients have limited awareness of what other services are potentially available in their local community to help them.

Mr. Berry added: "Partnering with Appello means we can design and develop innovative care packages with Local Authorities, Health and Community Trusts and Clinical Commissioning Groups to help discharge patients sooner, improve their outcomes and increase hospital capacity. Also we are looking to help reduce the instances of readmissions"
